Online Dental Consultation Consent
Effective date: 14 September 2026
The 15-minute online dental consultation is provided by Dr. Shubhrabaran Das, BDS, for a fee of ₹299, through a WhatsApp video or audio call or another method communicated to you.
The patient or guardian understands and voluntarily agrees that
- The consultation will take place remotely and not in person.
- The patient must give accurate information about identity, symptoms, current medicines, allergies and relevant medical history.
- An online assessment has limitations, because the dentist cannot examine the mouth physically.
- A final diagnosis may require a physical examination, X-ray, biopsy or another investigation.
- The consultation cannot independently confirm or rule out oral cancer or any other serious condition.
- No physical dental treatment or procedure can be carried out online.
- The dentist may advise urgent or routine in-person care at a clinic or hospital.
- A prescription will be given only when it is clinically appropriate and legally permissible.
- No diagnosis, cure, pain relief or treatment result is guaranteed.
- Relevant consultation and communication records may be maintained by the clinic.
- Necessary appointment communication may take place through WhatsApp or phone.
- Consent may be withdrawn before the consultation begins, subject to the cancellation policy.
- A patient below 18 years of age must be accompanied by a parent or lawful guardian.
This service is not for emergencies. Difficulty breathing or swallowing, rapidly increasing facial swelling, uncontrolled bleeding, serious facial injury, loss of consciousness or another emergency requires immediate in-person care.
By completing payment and participating in the consultation, the patient or guardian confirms that they have read and accepted this consent.
